Voices of Wellness and Care Coordination
Chronic disease afflicts millions of Americans, diminishing quality of life and placing a profound financial strain on our health care system. But many have fought back, participating in wellness and care coordination programs to change unhealthful behaviors, improve their health status and ward off ailments related to their chronic conditions. Their stories are here, in "Voices of Wellness and Care Coordination," a project of DMAA: The Care Continuum Alliance.

DMAA represents the many organizations and individuals dedicated to helping people prevent and manage chronic conditions. The evidence base for population health improvement programs, such as the ones profiled here, make a convincing case for incorporating wellness and care coordination practices throughout our care delivery system.
